Job Description:
Crossbridge Hospice is seeking a dedicated and empathetic Social
Worker to join our team in providing holistic end-of-life care. The
selected candidate will work closely with patients who have
life-limiting illnesses and their families to ensure their
emotional and social needs are met, in line with our mission to
enhance quality of life. This role demands a compassionate approach
and a strong ability to coordinate with interdisciplinary teams to
deliver comprehensive care plans tailored to individual needs.
Assess the psychosocial status of patients and families/caregivers
related to the patient's terminal illness and environment and
communicate findings to the registered nurse and other members of
the interdisciplinary group. Provide an assessment in the patient
and family/caregiver's psychological, emotional, and financial
needs. Plan and implement interventions that address the identified
needs. Educate patients and families on, and assist in, the
preparation of advanced directives and other necessary
documentation specific to their life-limiting illness. Assist
patients and families/caregivers with navigating the complexities
of medical and non-medical services. Facilitate needed discussion
regarding end-of-life issues and legacy planning. Document patient
care using a standardized method that relates to the care plan
goals. Guide the family through the patient's condition, treatment
decisions, and end-phase of life. Empathy and emotional
intelligence Solid understanding of healthcare and palliative care
frameworks Strong communication and interpersonal skills
Problem-solving and crisis intervention skills Ability to
collaborate effectively within an interdisciplinary team Medical

Licensed Clinical Social Worker (LCSW) or equivalent state-specific
licensure CPR Certification preferred
